Your subscription and additional appointment fees are set out in the Subscription Fee Schedule, which forms part of your Hybrid Subscription Agreement. You choose how you’d like to pay.
Billing options. You may elect annual, quarterly, or monthly billing. The three options reflect different total annual amounts — annual prepayment is the lowest, monthly the highest.
Annual. The full annual fee is paid once, up front, at the lowest total.
Quarterly installments. If you elect quarterly billing, you authorize the practice (or its billing administrator) to charge one-quarter (¼) of the applicable annual fee at the start of each three-month billing period during your subscription term. Quarterly billing is a convenience option and results in a higher annual total than annual prepayment.
Monthly installments. If you elect monthly billing, you authorize the practice (or its billing administrator) to charge one-twelfth (1/12) of the applicable annual fee at the start of each monthly billing period during your subscription term. Monthly billing is a convenience option and results in a higher annual total than both annual prepayment and quarterly billing.
Recurring payment authorization. By selecting quarterly or monthly billing, you authorize recurring charges to the payment method on file for each scheduled installment until the subscription is cancelled or the agreement is terminated in accordance with its terms.
Failed or late payments. If any installment is returned, declined, or unpaid, the practice may re-attempt the charge, assess a reasonable administrative fee, and may suspend or terminate services if amounts are not brought current within the time period specified in your agreement.
The Fee Schedule may be updated by the practice from time to time in accordance with the Hybrid Subscription Agreement.
